EazeHealth, led by Reamohetse Mokoatle, uses this website to understand interest in tools for South African healthcare practices and to invite suitable practice contacts to research or a potential pilot.
What we collect
The Google Form collects the professional contact details, practice details, preferences and contact-consent choice you submit. Google Forms also records the submission time. Please review the questions before submitting.
Please provide only your own professional contact information. Do not submit patient names, medical histories, symptoms, consultation recordings or other sensitive health information. This site is for registering practice interest, not receiving care or recording patient information.
Why we use it
With your consent, we use your details to assess demand, understand practice preferences and contact you about research or the founding pilot. Registration is voluntary, involves no payment and does not guarantee a pilot place.
Where responses go
New registrations are submitted directly to Google Forms and are available to the EazeHealth form owner through Google’s service. This landing page does not copy those responses into its own database. We do not sell your responses.
The embedded form loads content from Google, which may process technical information and use cookies under its own privacy policy. Our website hosting providers also process technical information needed to operate and protect this site. Registrations made through the earlier website form remain in the Sites-hosted database until removed.
How long we keep it
Signup records remain in the interest list until they are removed. This site does not currently apply automatic deletion. You can request removal as described below.
Your choices and deletion requests
For a Google Forms response, use “Contact form owner” in the form’s help menu to ask for correction or deletion, or reply to an EazeHealth research or pilot email you receive. Include enough information to identify your response, such as the email address used to register; do not send patient information.
You can withdraw contact consent by replying to an EazeHealth research or pilot email. If you registered through the earlier website form and saved its private removal link, that link still deletes the earlier website record. It does not delete a Google Forms response.
Measurement and technical information
Interest can be measured from responses in Google Forms. Campaign source tags on this website are not automatically added to Google Forms responses. EazeHealth has not added separate advertising or visitor-analytics tools. Google and the website hosting providers may process technical information for delivery and security.
